Profile Forms

The Parish and Minister’s profile forms are used during the pastoral assignment process. Both forms are available as:

  • A PDF form, which you’ll need to fill out by hand.
  • A Microsoft Word form, which you can fill out on your computer and then email to the appropriate person.

Parish Profile

Use the Parish Profile Form to describe all aspects of your church. This form will be given to prospective pastors.

Minister’s Profile Form

The Minister’s Profile Form gives a thorough picture of a minister. This form is required of persons applying for a position in the United Brethren church.
