The Parish and Minister’s profile forms are used during the pastoral assignment process. Both forms are available as:

A PDF form, which you’ll need to fill out by hand.
A Microsoft Word form, which you can fill out on your computer and then email to the appropriate person.

Parish Profile

Use the Parish Profile Form to describe all aspects of your church. This form will be given to persons being considered as senior pastor of your church.

The Minister’s Profile Form gives a thorough picture of a minister. Persons applying for a position in the United Brethren church must complete this form.

Ministerial Reference for Pastoral Transitions

Persons applying for a pastoral position in a United Brethren church must provide references from five persons: faculty, pastor, key layperson, cluster leader, and friend/family. The form below is used for that purpose.

Persons applying for a national conference license and ordination must provide references from five persons: faculty, pastor, key layperson, cluster leader, and friend/family. The form below is used for that purpose.
